A simple one-page form,. WDVA 2111, Pre-registration for Cemetery Interment can be filled out in our office. Interested veterans must be registered for benfits in the state system.

Cemetery facilities include the new administration-chapel building, outdoor committal chapel, ceremonial flag plaza and columbarium wall. MILITARY FUNERAL HONORS PROGRAM
To provide military funeral honors for Wisconsin veterans The Military Funeral Honors Program in Wisconsin provides a final tribute to the men and women who served our stale and nation honorably as members of the U.S. armed forces. It allows a grateful nation to pay its last respects in a dignified and solemn ceremony. The Wisconsin Department of Veterans Affairs administers the program with assistance from veterans' service organizations and the active and reserve components of the U.S. armed forces.

On the death of the veteran family members should contact their funeral director of their desire to have military funeral honors rendered. A DD214, the military discharge document will be needed and it simplifies the process if the family can locate it at home. Once necessary information is gathered the funeral director should then contact the State Veterans Department's Military Funeral Honors Program at 1-877-944-6667
