Home Visiting Programs for Families
The City of Milwaukee Health Department offers 3 different home visiting programs to meet the unique needs of Milwaukee's families. These home visiting programs hope to improve prenatal, birth, and/or infant outcomes for the families served through:
- Improved preventive prenatal and infant care
- Timely prenatal and infant health information
- Connection to community resources and support
What are the MHD's 3 Home Visitation Programs?
Empowering Families of Milwaukee
Empowering Families in Milwaukee (EFM) uses evidence-based models and partners with the community to provide frequent and long-term home visits to families. The focus of EFM is pregnant women and their children who reside in zip codes: 53204, 53205, 53206, 53208, 53212, and 53233.
Nurse-Family Partnership
Nurse-Family Partnership (NFP) is a research-based nurse home visiting program based on the David Olds model that provides frequent and long-term home visits to families. The focus of NFP is pregnant women and their children who reside in zip codes: 53204, 53210, 53212, and 53218.
Public Health Team Home Visiting
Public Health Team Home Visiting provides assurance in the community for mothers and infants through short-term home visits. The focus of Public Health Team Home Visiting is pregnant teens, high-risk infants, and children who need additional health and developmental assessment citywide.
