FAQ - Single Coordinated Care Plan (SCCP)

 What is a SCCP?
A Single Coordinated Care Plan is a strength-based point of service plan crafted in a team meeting attended by multiple systems (government, community, faith, social, family, etc.). The client identifies needs and sets goals he or she wants to achieve in the next 6-12 months. As part of the strength-based service plan, individuals in the team meeting may offer to support the client in achieving those goals. The plan is updated as goals change or needs are met.
 Why are they useful?
A SCCP is successful because the client creates it with the support of groups, agencies, and friends to reach goals they have set for themselves. It is useful for setting clear and consistent guidelines for recovery and as a tool for measuring progress toward recovery.
 Who may fill out a SCCP?
Recovery Support Coordinators, Care Coordinators, and Bureau of Milwaukee Child Welfare Workers may fill out a SCCP.
 How often is a SCCP updated?
A SCCP must be completed for a client within 30 days of service. After creation, the plan should be updated every 60 days, but it may be changed more often, if needed.
 Does a client have to have a Single Coordinated Care Plan?
In the ATR Program, every client needs to have a SCCP as part of Recovery Support Coordination.
